Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazard(s) that were causing or were likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to struck by or crushing hazards by moving vehicles: a)At the worksite - A JLG Model 80HX aerial lift was not positioned on a level surface which caused it to tip over when the boom was raised; on or about 07/05/11. Among other methods, some feasible and acceptable abatement methods to correct this hazard are to: 1) As per the JLG Operation and Safety Manual, User Responsibilities and Machine Controls, Operating Characteristics and Limitations involving Stability Control; operate the machine within its rated capacity on a smooth, firm and level supporting surface which provides a stable machine for all platform positions. 2) Utilize figure 3-1 and 3-2 in the Safety Manual for machine stability for Forward Stability and Backward Stability. And 3) Ensure all machine systems are functioning properly. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $ 1,000.00 AS PER 29 CFR 1903.19.
