where risk is apparent or may be anticipated. Arrange for a reliable response system when an alarm is triggered. The use of administrative controls i.e. locking staff offices during the night shift, implementation of a buddy system, determine the behavioral history of new and transferred patients to learn about any past violent or assaultive behaviors, establish a system-such as chart tags, log books or verbal census reports---to identify consumers with assaultive behavior problems. Keep in mind confidentiality and worker safety issues and update as needed. Keep a log book and reports of such incidents to help determine effectiveness of workplace controls and make any necessary changes to prevent reoccurrences. Follow written security procedures in order to prevent employee exposure to workplace violence. (3) Ensure continuous security presence/personnel on facility floors at all times (mainly evening and night shifts). (4) Ensure security presence/personnel on floors during the administration of medication to consumers. (5) Establish open communication between security guards and Institute for Community Living Inc. employees to help identify problematic consumers and recent situations which could result in an incident. (6) Security personnel should be trained in ICL crisis intervention and workplace violence prevention policies and programs. (7) Annual review and evaluation of workplace violence policies, programs and controls. Employer will certify the annual review and set a goal to reduce incidences of workplace violence. (8) The development and training of employees on an emergency communication plan. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $ 1,000.00 AS PER 29 CFR 1903.190
