Text For Citation: 01 Item/Group: 001 Hazard: CAUGHT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught in and/or caught by in-going nip points on conveyors: (a) Location - Pit Area, Barclay shredder: On or about 05/15/09, the emergency stop switch for the Barclay shredder had a broken stop button rendering it not operable. (b) Location - Pit Area, 4" Cross Conveyor: On or about 05/15/09, the emergency stop switch for the cross conveyor at the Columbus McKinnon 4" shredder was not operable. Feasible abatement methods include but are not limited to the following: 1. Ensure compliance with ASME B20.1-2003 Safety Standards for Conveyors and Related Equipment, section 5.11.2 for emergency stop devices and comply with the manufacturers' design for emergency stop devices. 2. Establish and enforce regularly scheduled inspection and operation checks to ensure employees follow a preventative maintenance and inspection program for conveyor safety devices and components. 3. Encourage employees to report any damage to or missing components that may compromise conveyor safety.
