Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to or struck by crushing hazards: a.Nexus Drilling Rig #2, Onsite, Camden, WV: The employer placed the opened "eye" end of a guyline over the end of a round steel bar located at the bottom of an 85,000 Lb. Cardinal Bender air compressor which is used when picking up the air compressor via a winch line when moving the air compressor from site to site. The guyline was not properly secured to the steel bar on the air compressor by any means. The end of the guyline was resting just 3-4 inches from the end of the steel bar as observed on or about 03/09/2010. Abatement: Among other methods, feasible and acceptable abatement methods to correct this hazardous condition would be to: 1) Follow IDECO BIR-3084 Rambler Rig Manual requirements on guying and guylines, and 2) Follow industry-wide recommended practice American Petroleum Institute (API) Recommended Practice (RP) 4G addressing safe guying and guyline anchoring practices. ABATEMENT CERTIFICATION REQUIRED FOR THIS CITATION.
