Text For Citation: 01 Item/Group: 001 Hazard: LIFTING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)Spud Cellar: On or about September 23, 2010 and at times prior thereto, employees were exposed to a crushing hazard when a 1974 Hyster H40H was used to elevate an employee approximately 9 feet to repair a door frame. The forklift was operated in an unsafe manner as it was moved to reposition the elevated employee. The operator was not properly trained or qualified to operate this fork lift. b)Spud Cellar: On or about September 23, 2010 and at times prior thereto, employees were exposed to a fall hazard when standing on an unsecured pallet supported by a 1974 Hyster H40H to repair a door frame. The platform did not have guardrails nor was the platform secured to the forks of the forklift. c)Spud Cellar: On or about September 23, 2010 and at times prior thereto, employees were exposed to the unguarded lifting mechanism of the forklift mast while standing on a pallet supported by a 1974 Hyster H40H to repair a door frame. Among other methods, feasible and accepted methods to correct these hazards are to comply with safety rules prescribed in the 1974 Hyster H40H operator manual and ANSI B56.1, Safety Standard for High Lift and Low Lift Trucks, including but not limited to: ensure operators are trained to operate forklifts, ensure platforms used to elevate personnel are secured to the forks, and ensure platforms used to elevate personnel are equipped with fall protective devices such as guard rails, and ensure that platform provides guarding from the in running nip points on the mast. Note: Abatement certification and supporting documentation are required for this item.
