Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards while a torching operation was being conducted: On or about 9/3/2009, at 1501 NE 62nd Street, Fort Lauderdale, Florida, employees were exposed to a struck-by hazard when an employee was using a torch to tap into Chiller Pump #2 to install a pressure gage and did not disconnect the temporary PVC by-pass attachment from the chiller unit. Among other methods, one feasible and acceptable abatement method to correct this hazard is to remove the temporary PVC by-pass connection pipes before torching.
