On or about June 2, 2009, and at times prior thereto, at the establishment located in the Highland Industrial Park on Highway 274 in Camden, Arkansas employees entered a permit- required confined space to perform an abrading procedure on the interior insulation of an Army Tactical Missile rocket motor and the employees used forced air ventilation. The ventilation line was supplied by a compressor that also supplied air to an on-site nitrogen generation operation. The ventilation line and nitrogen generation equipment were connected to a common pipe header and there was a malfunction in the equipment designed to prevent nitrogen from back feeding into the ventilation line. This created a condition that exposed the employees to the hazard of asphyxiation due to an oxygen deficient atmosphere.
