Section 5(a)(1) of the Occupational Safety and Health Act of 1970: Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Location:Movie set gantry, Scenic Artist Lye Area Room, and temporary Chrysler building. Employees did not follow safe ladder practices; on or about 04/07/09. a)Employees did not climb up and down facing the ladder. b)Employees used an extension ladder used to access the crane did not extend three feet above the landing. c)An employee used an A frame ladder leaned on a wall. d)Employees used the top cap and top step of A frame ladder. Among other methods, a feasible and acceptable abatement method to correct this hazard is: 1)Reference: ANSI A14.5- 2000, Paragraph 9.3.7: "When ascending or descending the ladder, the user shall face the ladder and maintain a firm hold on the ladder." 2)Reference: ANSI A14.5- 2000, Paragraph 9.3.10: "When single section or extension ladders are used to get on or off at upper levels, the top of the ladder shall extend approximately three feet above the point of support at eaves, gutter or roof line." 3)Reference: ANSI A14.5- 2000, Paragraph 9.3.13.2: "The user shall ensure that the step ladder is fully opened, with spreaders locked and all feet contacting a firm level support surface. 4)Reference: ANSI A14.5- 2000, Paragraph 9.3.2: "The user shall not step or stand on (1) A ladder top cap and the top step or trestle ladder, or bucket or pail shelf of a self supporting ladder. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19NCE
