Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of overexposure to anhydrous ammonia: (a) Location - Engine Room: On or about 4/16/09, low pressure receiver V7 was not inspected as often as required by an outside party. (b) Location - Engine Room: On or about 4/16/09, the company did not have information about low pressure receiver V7, including but not limited to Manufacturer's name, serial number, model number, maximum allowable pressure, and test pressure applied. Among other methods, a feasible method to eliminate or reduce this hazard is to have an inspection conducted of the receiver at least once every five years by an outside party, as mandated by International Institute of Ammonia Refrigeration (IIAR) Bulletin No. 110, Guidelines for Start-up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems, Section 6.4.4.1. The insulation may be removed to allow name plate inspection, with subsequent repair and insulation plug provided to cover name plate area and to provide future access, as (IIAR 109, Guidelines for IIAR Minimum Safety Criteria for a Safe Ammonia Refrigeration System, Sections 4.3.1.2 and 4.3.4).
