Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck-by an excavator's bucket due to the excavator not being shut down prior to the operator leaving the excavator's cab. a) Site - City of Auburndale Wastewater Treatment Plant - Daewoo Excavator - Model # 290LC-V - Serial # 809. An employee was struck-by an excavator's bucket while attempting to attach a wire rope sling around a concrete meter vault, on or about, 01/29/09. Feasible Means of Abatement, Among Others, Include: a. Require all operators to shut down the excavator prior to leaving the excavator's cab and enforce the requirement. b. Have all operators conduct daily inspections of all operational controls to ensure that the safety lever and micro switch is operational. c. Conduct unannounced inspections to ensure that all operators are following all safety procedures during the operation of the excavator.
