Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being caught between/crushed. On or about 02/28/2010, the employer did not ensure that the operator of the Iron Roughneck understood the operation of the National Oilwell Varco, Model # IR-3080 Iron Roughneck (IR) that was in use on Rig 101. This exposed employees to hazards related to being caught by/crushed by. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to: provide training or refresher training to IR operators and verify that they have understood the training provided to them.
