29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section: The employer does not ensure that energy control procedures clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy when removing the heads off heat exchangers. The violation most recently occurred at the employer's facility located at 2401 5th Ave S, Texas City, TX on or about September 21, 2010 in the Pipestills 3B Unit, where pipefitters performing maintenance on the heat exchanger 427-C. Employees were exposed steam at high temperatures and struck by hazards.ures
