Nevada Revised Statute 618.375(1): The employer failed to furnish employment and a place of employment which was free from recognized hazards that were causing or were likely to cause death or serious physical harm to his employees: The employer failed to furnish employment and a place of employment which was free from recognized hazards for employees working in the kitchens at the MGM Grand Hotel and Casino, as follows: a.Inside the MGM Grand Hotel and Casino, the maintenance for the kitchen equipment was not done in accordance with the Manufacturer's manual. The pilots were not adjusted properly; accumulation of gas builds up causing either an improper combustion or small explosion while turning the knobs to light the burners in the following kitchens: 1. Inside the Bakery, one (1) pilot of the JR range was out. The knob was turned on but the burner did not ignite. 2. Inside the Room service kitchen, two Southbend stoves had six (6) and four (4) burners that did not ignite when the knobs were turned on and one (1) pilot was out. Also at the Southbend salamander the flame was not steady and was on and off. 3. Inside the Grand Wok, at the wok line two (2) pilots were not working. The pilots did not ignite. 4. At the Pearl Restaurant main kitchen, eight (8) burners had the pilot out. A gas smell was detected as well as the kitchen at the west corner of the Pearl Restaurant. Four (4) pilots were not lit. 5. Inside the Seablue Restaurant, it was observed a Chef turning the knobs of the Vulcan flat top. While he was turning the knobs on, an explosion occurred. Large flames shot from under the flat top. 6. At the L'Atelier de Joel Robuchon kitchen, a gas smell was detected at the north west side of the kitchen stove. The pilots were not on. 7. Inside the Studio Cafe kitchen, two (2) burners on the Southbend stove were not working properly. At the time that the Chef turned the knobs on, the burners did not ignite. Reference ANSI (NATIONAL FUEL GAS CODE) Z223.1 -1974 Section 2.7.2.4 Adjustments: Adjustments to burners, pilots, ignition devices, protective devices and controls shall be made in accordance with manufacturer's instructions and applicable safety requirements. Section 2.8.1.1. A maintenance program shall be established. This program should include adherence to manufacturer's recommendations. In this program a minimum maintenance schedule should include inspection and action on the recommendations outlined under A.3 in Appendix 2-A. One feasible means of abatement is to follow the manufacturer's recommendations.
