Nevada Revised Statute 618.375(1): Every employer shall furnish employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees: The employer failed to ensure that automotive lift operators were properly trained, as follows: a.At Boulder Creek Golf Course maintenance shop, Employee #1 was exposed to hazards while operating an automotive lift (make: Bend Pak, model no: HD-9, serial no: 6814) to perform maintenance on golf course maintenance equipment without being properly trained. Reference: ANSI/SIA ALOIM-2000 Standard for Automotive Lifts - Safety Requirements for Operation, Inspection, and Maintenance, Section 4.2 Operator Training. 4.2 The owner or employer shall ensure that operators of automotive lifts are instructed in the safe use and operation of the lift using the manufacturer-provided instructions and warning labels and the Automotive Lift Institute publications, "Quick Reference Guide, Vehicle Lifting Points for Frame Engaging Lifts", "Lifting It Right", "Safety Tips" and this ALOIM Standard. b.At Boulder Creek Golf Course maintenance shop, the employer did not appropriately document training for Employee #1 who was permitted to operate the automotive lift. Reference: ANSI/SIA ALOIM-2000 Standard for Automotive Lifts - Safety Requirements for Operation, Inspection, and Maintenance, Section 4.2 Operator Training. 4.3 Operator Training Documentation. The owner or employer shall appropriately document operator training by completing the operator training log found in Appendix - A to this standard.
