Text For Citation: 01 Item/Group: 001 Hazard: CONFINED

Section 5(a)(1) of the Occupational safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards associated with working in a confined space: a. On or about July 29, 2009, at a jobsite located at 1510 W. Cortez Street, Chicago, Illinois - employees were performing carpentry work from a platform (jobmade wooden box) on the forks of a rough terrain forklift truck. The operator did not remain in the cab to operate the controls, while employees were elevated in the platform. The platform in which the employees were working was not equipped with controls. Among other methods, one feasible and acceptable means to correct these hazards are to use a safer alternate method such as: Follow the American National Standard Safety Standard for Rough Terrain Forklift Trucks ASME B56.6-2002, Section 5.15(s) a trained operator shall be in position to control the rough terrain forklift truck, or available to operate controls if the platform is not equipped with controls.
