Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: a)On or about June 28th, 2010 employees were installing metal roofing members using a Simon 2515RT scissors lift to ascend the roofing structure. The employees were climbing onto and over the top and mid-rails of the Simon scissors lift, exposing them to a fall hazard of thirteen and a half (13.5) feet from the scissors lift railing to the ground level. Among other methods, feasible and acceptable means to correct these hazards are to use other equipment with which to access the roofing structure and to ensure the Simon scissors lift is operated according to the Simon Operator's Manual and acceptable requirements of the approved American National Standards for Self- Propelled Elevating Work Platforms ANSI/SIA A92.6-2006, Section 7 Responsibilities of User, and other acceptable industry practices. The ANSI consensus standard requires the following: 7.10Operator warning and instructions (9) Personnel footing. Personnel shall maintain a firm footing on the platform floor while working thereon. Climbing by occupants on the midrails or toprails of the aerial platform is prohibited. The use of planks, ladders, or any other devices on the platform for achieving additional heights or reach prohibited. No Abatement Certification or Abatement Documentation required.
