Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to flame jets, fire balls, and struck-by hazards: (a) On or about 10/06/2008 - Located at the Brookwood Methane Field - Gob Well Number JWR14-15-3 - Roustabout employees charged the separator system with flammable methane gas from the well at unregulated pressures to check its piping and connections for leaks resulting in system over pressurization and methane release at ground level. Among other methods, one feasible and acceptable method to correct this hazard is to conduct leak tests using air or other recognized and generally accepted method and prohibit blocking off vent stacks when the wellhead is opened:
