Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by and caught in-between hazard from the steel/metal storage racks not being anchored to the floor: On or about April 1, 2009 the employer's representative was using the forklift to store manufactured hose products and other types of storage items such as parts and equipment for the manufacturing process. The storage racks sit at an approximate height of 10-14 feet and were not anchored or secured to the floor. The employer runs a very high forklift traffic operation and the operators were not trained in the operation of the forklift which could cause operator error and collapse of the racks. One feasible method and acceptable method to correct this hazard would be to follow the installation instructions and the ANSI standard MH16-1-2004 referencing the anchoring of the storage racks. No abatement certification or documentation is required for this item.
