Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: A.) On or about 9/23/08, employee were using a spreader bar on a hoist that had not been rated for its approved load, tested or marked for its intended load. Maintenance employees fabricated the attachment on site but the employer did not assure its testing or marking. Employees use the bar to lift scrim rolls onto the airlay line. B.) On or about 9/23/08, employees were using a hook that was missing a pin for lifting and retrieving water jet equipment (that is typically stored outside) and press equipment. The hook had not been inspected and/or taken out of service. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure the hook and spreader bar are proof testing, inspected and/or marked in accordance with ASME B30.20-2006, Below-the-hook Lifting Devices.
