Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment or a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by, and/or crushed by, the movement or falling of Tilt- Up Wall Panels. On 2/3/08 at the LA Fitness building construction site, employees were involved with the construction and erection of Tilt-Up Wall Panels during the construction of the building. An outside wall panel (Panel #24) was being erected in front of a previously erected inside wall panel (panel #23). The outside wall panel made contact with the inside wall panel causing the inside wall panel to fall to the inside of the building and to the concrete floor. When the outside wall panel was cast, a concrete top (Bumpout) was included during the casting of the panel and this concrete top was not shown in the original Tilt-Wall Panel Book Specifications, and it was not shown on the revised Tilt-Wall Specifications that were engineered for the new two point pick points. One feasible and acceptable abatement method to correct this hazard is to follow the Warning Instructions that are in Universal Building Products Tilt-Up Erection Engineering Panel Book which state "do not deviate from the information shown in the drawings or Universal Building Products Tilt-Up Technical Manual without notifying, and obtaining approval from, Universal Building Products Engineering Department".
