Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)At the job site, on or about 02/19/2010: Employees were exposed to being struck by the equipment when they were operating mobile refuse compactor equipment without adequate training and signage. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1.Provide instruction and training to employees in the safe methods of work before assigning them to operate, clean, service, maintain or repair the equipment. The instruction or training shall include the procedures provided by the manufacturer. ANSI Z245.1 - 1992, Section: Operational requirements, para: 6.1(a), Mobile Refuse Collection and Compaction equipment - Safety Requirements. 2.The employer will maintain records of the names of employees trained and the dates of training, ANSI Z245.1 - 1992, Mobile Refuse Collection and Compaction equipment - Safety Requirements, Section: Operational requirements. para: 6.1(a). b)At the job site, on or about 02/23/2010: Employees were exposed to being struck by the mobile refuse truck (Mack License #27513-JU) that was lacking an operational external audible alarm when the vehicle was operated in reverse. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: Repair or replace the external audible alarm with a minimum output of 87 db. ANSI Z245.1 - 1992, Mobile Refuse Collection and Compaction equipment - Safety Requirements, Section: Operational requirements, para: 7.1.15.2. Abatement certification must be submitted for these items.
