Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Stack Melter area, on or about 10/08/2009: A locally fabricated vertical Jib rail was used to support a 1/2 ton electrically operated chain hoist had not been initially tested, rated and marked to determine if it would withstand the forces imposed by the hoist/rated load and was never inspected with records maintained. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1.Test, rate, and mark the jib rail to ensure it will withstand the forces generated by the hoist and the rated load. ANSI B30.11.1-1980, Monorails and Underhung Cranes, Section 11.1 Construction and Installation. 2.Conduct frequent and periodic inspections of the wall supported jib rail and maintain records in accordance with ANSI B30.11.2-1980 Monorail and Underhung Cranes, Section 11-2 Inspection, Testing, and Maintenance. b)Floor molding, on or about 10/08/2009: 3 locally fabricated, 7-feet long, metal spreader below-the-hook hoisting beams had not been initially tested, rated marked and no inspections or external code mark record(s) was used to determine external conditions. Among other methods, feasible and acceptable abatement methods to correct this hazard are to: 1.Rate and mark the load capacity of the lifting devices on a tag/plate or equivalent. Section 20-1, ANSI/ASME B30.20-1993, Below-the-Hook Lifting Devices. 2.Conduct frequent and periodic inspections of the lifting device in accordance with Section 20-1, ANSI/ASME B30.20-1993, Below-the-Hook Lifting Devices. c)Stack melter area, on or about 10/08/2009: A locally fabricated below-the hook lifting device was attached to a 1/2 ton Centurion, overhead electrical chain hoist that had never been tested, rated or inspected with maintenance records maintained. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1.Rate and mark the load capacity of the lifting devices on a tag/plate or equivalent. Section 20-1, ANSI/ASME B30.20-1993, Below-the-Hook Lifting Devices. 2.Conduct frequent and periodic inspections of the lifting device in accordance with Section 20-1, ANSI/ASME B30.20-1993, Below-the-Hook Lifting Devices. d)Stack melter area, on or about 10/08/2009: A 1/2 ton Centurion, overhead electrical chain hoist, had the hoist hook locally modified by closing the throat of the hook with a metal bolt welded across the opening and was never tested, rated or inspected following the modification. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1.Rate and mark the load capacity of the lifting devices on a tag/plate or equivalent. Section 20-1, ANSI/ASME B30.20-1993, Below-the-Hook Lifting Devices. 2.Conduct frequent and periodic inspections of the lifting device in accordance with Section 20-1, ANSI/ASME B30.20-1993, Below-the-Hook Lifting Devices. Abatement certification must be submitted for items a,c & d only.
