Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Parts Wash area, on or about 09/10/2009: A CM 1/2 ton, Lode star Overhead hoist did not have a safety latch on the hook, the lower limit switch was not properly adjusted in accordance with the manufacturer's manual and no inspection and maintenance records were being maintained. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1.Repair or replace the safety latch and the lower limit switches and ensure they are adjusted to meet the requirement of the manufacturer's operation manual, Page #18: Setting limit switches and ANSI B30.16.-1993, Overhead Hoists, Section 16.2.3.3 Adjustments, Repairs, and Replacements. 2.Conduct frequent and periodic inspections of the hoists in accordance with ANSI B30.16-1993 Overhead Hoists, Section 16-2.1 Inspection. Abatement certification must be submitted for this item.
