Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Back room, on or about 06/18/2009: A 12-inch metal beam rail locally fabricated and installed to support two overhead hoists that were used to lift paper machine rolls ranging in weight from 1500 lbs to 8000 lbs. was not tested, rated or inspected. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1.Mark the load capacity on the metal beam rail in accordance with ANSI B30.11-1980, Monorail Systems and Under Hung Cranes, Sections 11.1, Construction and Installation. 2.Inspect and test the metal beam rail in accordance with ANSI B30.11 - 1980, Monorail Systems and under hung cranes, Section 11.2. Inspection, Testing, and Maintenance. Abatement certification must be submitted for this item.
