Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being Struck by and/ or Crushed: a)On or about 5/30/09, In the Garage, Truck #310: There was no positive means of support for the dump body. Employees were not provided with training regarding the safe operation of vehicles. Employees were not periodically monitored while performing work. b)On or about 6/10/09, In the Yard, Truck #337: The hoisting cable was crushed and deformed, there was no operable hook latch, and there was no cable thimble at the hook attachment. c)On or about 6/10/09, In the Yard, Truck #347: There was no hook latch on the hoisting hook. d)On or about 6/10/09, In the Yard, Truck #344: The hoisting cable was crushed. Among other methods, feasible and acceptable abatement methods to correct these hazards are to: 1.Install and maintain positive means of support for the dump bodies of all vehicles equipped with dump bodies, as set forth in American National Standards Institute (ANSI) Z245.1, Mobile Refuse Collection and Compaction Equipment Safety Requirements, section 7.1.5. 2.Ensure that employees are trained on safe methods of performing their work, as set forth in American National Standards Institute (ANSI) Z245.1, Mobile Refuse Collection and Compaction Equipment Safety Requirements, sections 5.1 and 6.1 a. 3.Monitor the employees operation of equipment and ensure that employees are adhering to safe practices, as set forth in American National Standards Institute (ANSI) Z245.1, Mobile Refuse Collection and Compaction Equipment Safety Requirements, section 6.1 b. 4.Perform regular inspections of all vehicles, including cabling and hoisting mechanisms, as set forth in American National Standards Institute (ANSI) Z245.1, Mobile Refuse Collection and Compaction Equipment Safety Requirements, section 6.1 c. Abatement documentation must be submitted for this item.
