Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY or CAUGHT IN: a)At the job site, on or about 12/31/08: Employees were exposed to being struck by or caught in equipment when they were servicing mobile refuse compactor equipment without adequate training and signage. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1.Provide instruction and training to employees in the safe methods of work before assigning then to operate, clean, service,maintain, or repair the equipment. The instruction or training shall include the procedures provided by the manufacturer. Section: Operational Requirements, para: 6.1(a), ANSI Z245.1 - 1992, Mobile Refuse Collection and Compaction equipment - Safety Requirements. 3.Replace or install cautionary safety signs on all vehicles equipped with bodies located next to all body entrance locations on the outside such as: "CAUTION - STOP ENGINE AND REMOVE IGNITION KEY BEFORE ENTERING", Section: Mobile equipment safeguards and features, para: 7.1.12, ANSI Z245.1 - 1992, Mobile Refuse Collection and Compaction equipment - Safety Requirements. b)At the job site, on or about 12/31/08: Employees were exposed to being struck by the mobile refuse compactor truck (MACK License# NY 15803JY) that was lacking an operational external audible alarm when the vehicle was operated in reverse. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: Repair or replace the external audible alarm with a minimum out put of 87 db.
