Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling from ladders: a)Tank Room 28, on or about 9/26/08: Tank #86, fiberglass extension ladder used at a near vertical angle. b)Old Tank Room, on or about 10/01/08: Tank #28, fiberglass extension ladder used at a near vertical angle. c)Tank Room 28, on or about 9/26/08: Fiberglass extension ladder was not extended and had no grab bar for employees stepping to and from the ladder. d)Old Tank Room, on or about 10/01/08: Fiberglass extension ladder was not extended and had no grab bar for employees stepping to and from the ladder. Among other methods, one feasible method of abatement is to: 1)Ensure that ladders are used as per ANSI A14.5-2000, Portable Reinforced Plastic Ladders, Section 9.3. Abatement certification must be submitted for this item.
