Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to struck-by hazards from unsecured well head connections: a) East Resources Becker 404 1H Gas Well/Tioga County/Jackson Twp.PA: Employees positioned too close to the well head during hydraulic fracking operations were exposed to struck-by hazards from a failed chixsen or elbow connecting long iron from the manifold trailer to the well head, on or about 11/06/09. Among other methods, one feasible and acceptable method to abate this hazard is to ensure wire line units are spotted as far away from fracturing units as practicable in accordance with American Petroleum Institute (API) RP 54 Recommended Practice for Occupational Safety of Oil and Gas Well Drilling and Servicing Operations Manual, page 24, section 13.2.1. ABATEMENT OBSERVED NO CERTIFICATION REQUIRED
