Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: a)Jobsite: Employees who were installing electric motor assemblies during the installation of worker's hoists at a construction site were exposed to being caught and crushed between the motor assemblies and the tops of the hoist cars. On or before 6/17/08. Abatement Note One method, among others, to eliminate or to materially reduce this hazard would be to comply with American National Standard A10.44-2006 Control of Energy Sources (Lockout/Tagout) for Construction and Demolition Operations and establish a program consisting of energy control procedures, employee training, and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated form the energy source and rendered inoperative. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
