Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by hazards while removing a rotating head stripper rubber while under pressure: (a) On or about May 15, 2008, for the employee removing the clamp on the rotating head stripper rubber without ensuring that the pressure was bled off, on Cyclone Rig #20, located at 5202 Highway 8, Stanley, North Dakota. Among other methods, one feasible and acceptable method to correct this hazard would be the implementation of an employee training program as described in Section 6 of the American Petroleum Institute - Recommended Practice 54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, section 6.1.6, which states, "Personnel should receive instruction in correct work methods to reduce chance of injury to themselves or fellow personnel."
