Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing; struck-against and being-ejected hazards; glass fragments; and poor visibility: On or about 12/10/2008, at 1600 Johnson Street, in Hollywood, Florida, a Chevrolet Cheyenne 3500 pick-up truck was driven with a cracked windshield and non-operable brake lights. Among other methods, a feasible and acceptable means of abatement is for the employer to follow the manufacturer's guidelines and restore the Chevrolet Cheyenne 3500 pick-up truck to its original safe condition; and to implement a policy stating that any device or element of design installed on the vehicle shall not be rendered inoperative, in whole or in part, without prior written approval from the equipment manufacturer. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
