Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and fall hazards: On or about 07/02/2008, at 1914 West Atlantiv Blvd., in Pompano Beach, Florida, a chain provided to secure an orange Ballymore personnel platform to the powered industrial truck was not used. On or about 07/02/2008, at 1914 West Atlantic Blvd., in Pompano Beach, Florida, the locking mechanism to an orange Ballymore personnel platform swing gate did not close with ease. Among other methods of abatement, one feasible and acceptable method to correct these hazards is to ensure compliance with the manufacturer's recommended practices for securing the platform and ensuring that the locking mechanism is in proper working condition. PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
