Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees using the freight elevator were exposed to fall hazard when the door opened without the car being present, and no periodic inspections had been performed which would have revealed this condition: (a)On or about 10/01/2008, the freight elevator door on the second floor was able to open without the car being present exposing the operator(s) to a fall hazard, and no periodic inspections had been performed, according to ASME A17.1. ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM Feasible means of abatement for this would be: 1. Conduct an inspection by a qualified elevator inspector and schedule periodic inspections according to ASME A17.1 2. Install interlocks to prevent the elevator door from opening when the car is not present according to ASME 17.3 3. Make any and all corrections revealed during the inspections 4. Instruct employees not to override safety mechanisms and not to use the elevator if there are defects.
