Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a)During preparation for installation of an underground storage tank, on or about 07/09/08: Employee was exposed to a falling or crushing hazard while riding/standing in the bucket of a John Deere 120C excavator during installation of a length of plastic culvert used for dewatering the excavation. Among other methods, one feasible and acceptable means of abatement would be to follow the manufacturer's safety instructions in the operator's manual which indicate death or serious injury may result from riding in the bucket of the excavator.
