Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: (a)Sears Auto and Tire Center at 205 E. Foothills Parkway, Fort Collins, CO 80525: On or before 1/16/09, the employer did not ensure that the Dayton 1,000 pound hoist, model 5W659, used to lift the tire conveyor, was free from the following recognized hazards: 1)The hoist did not provide for over-travel protection in accordance with the American Society of Mechanical Engineers (ASME) standard B30.16 - 1993 for Overhead Hoists (Underhung) Section 16-1.2.14: "The hoist shall be so designed and constructed that the load hook, either loaded or empty, shall not exceed the upper limit of travel." 2)The hoist braking system did not stop and hold the load hook when the controls were released in accordance with ASME B30.16 - 1993 Section 16- 1.2.11(b)(1)(a); 3)The supporting structure was not designed to withstand the loads and forces imposed by the hoist for the rated load in accordance with ASME B30.16 - 1993 Section 16-1.3.2. The hoist was anchored to some angle iron used to support some heaters, and there was no assurance that this anchorage point could support the tension placed upon it by running the hoist load hook into the drum, along with the weight of the load, and the weight of the heaters; 4)The hoist did not include a marking to depict its rated load on the hoist or load block in accordance with ASME B30.16 - 1993 Section 16-1.1.1; 5)The hoist was not frequently inspected in accordance with ASME B30.16 - 1993 Section 16-2.1.2, at least monthly; 6)The hoist was not periodically inspected in accordance with ASME B30.16 - 1993 Section 16-2.1.3, at least annually, and who makes records of external conditions to provide a basis for a continuing evaluation in accordance with ASME B30.16 - 1993 Section 16-2.1; and 7)The hoist was not centered over the load except when authorized by a qualified person in accordance with ASME B30.16 - 1993 Section 16-3.1.2.
