Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: (a)Halliburton at Leverich 31-13B, near Rifle, CO 80202: On September 8, 2008 the employer did not ensure the safety of employees on site in that the controls of the AMS4A046 winch operators panel were not adequately monitored by the operator of the winch. The depth alarm was set for 500 feet, however steps were not taken to activate the line tension adjustment valve or other measures to control the speed of the wireline. This condition exposed two operators to a struck by hazard as the tools came up out of the well head, hit the sheave, and consequently fell. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to perform the following: Revise and clarify the current Risk Analysis Chart for Winch Operations to, among other things, require the use of the Company's Depth and Tension Record ("Z-chart") and to add more detail on the safe use of the line tension adjustment valve. All winch operators in the State of Colorado shall be trained on the revised Risk Analysis Chart.
