Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards during boom disassembly (de-rigging): a)On April 21, 2008, at HPU 3-3 #2 drill site, Creola, Al: During the de-rigging operation of the Link-Belt LS 138H II crawler crane, an employee was underneath the unsupported crane boom knocking out the pins that connected the 20 foot and 30 foot sections, and did not support the crane boom with cribbing, exposing the employee to being crushed by the collapsing boom. Among other methods , feasible and acceptable abatement methods to correct this hazard are to: 1) Follow and keep a copy of the operator's manual for the Link-Belt LS 138H II crawler crane at the job site, which requires in section 4, page 4- 41 through 4-43 to be followed during de-rigging. 2) Ensure that employees involved in the de- rigging operation are properly trained to follow the Link-Belt LS 138H II crawler crane operator's manual when dismantling the crane boom sections. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED.
