Text For Citation: 01 Item/Group: 001 Hazard: HEAT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to accumulated fuel gases and potential fire, and oxygen deficient atmospheres. a) Peacock Oil Company, South Equipment Room - Gas utilization equipment including: Raypak Heating Boiler, Model 724 WT-B, Serial Number 0988106022; Raypak Heating Boiler, Model 516T, Serial Number 1278103800TP; and, Dayton Unit Heater, Model 3E226A, Serial Number C8412734, installed in buildings: Required air was not supplied for combustion and ventilation, nor for comfort and proper working conditions for personnel. One feasible means of abatement, among others is to provide the space with two permanent openings to the outdoors, one commencing within 12 inches of the top and one commencing within 12 inches of the bottom of the enclosure and which conforms to NFPA 54 - National Fuel Gas Code. NFPA 54 (1988) states that the "openings shall communicate directly or by ducts, with the outdoors or spaces (crawl or attic) that freely communicate with the outdoors. When directly communicating with the outdoors, for example, each opening shall have a minimum free area of 1 square inch per 4000 BTU per hour of total input rating of all equipment in the enclosure." (Chapter 5.3.3) In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
