Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Section 5(a)(1) of the Occupational Safety and Health Act of 1970: employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and amputation hazards caused by the unintentional activation of foot pedal: a) On or about 1/25/08 in the shop area, employees were exposed to an amputation hazards from the unintended activation of the press brakes. The foot pedals were unguarded or a inadequately guarded on press brakes such as, but not limited to the Di- Acro Hydra-Power Press Brake(beige in color) serial # 6820882871, model # 14-48-2 and Rousselle Stamping press 4E EES 9471. One feasible and acceptable abatement method to correct this hazard: 1) Guard the foot pedals as referenced in ANSI B11.3 1982, Safety Requirements for Power and press brakes Section 3.24 and 3.25. ABATEMENT DOCUMENTATION REQUIRED
