Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to the employees in that employees were exposed to struck by and crushing hazards: (a)Nexus Drilling, Rig #4, Daisy Ross No. 2 well site - Employees were performing work on and around the drilling rig. Employees were exposed to the hazard of collapse of the derrick/mast in that the employer did not ensure that the guy wire anchor spacing/pattern was consistent with the manufacturer's recommendations or with consensus standards, as determined on 03/31/09. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow accepted safe practices such as but not limited to; a) install guy wire anchors according to the rig manufacturer's recommendations for guying patterns, or b) install guy wires according to the Recommended Practice for Use and Procedures for Inspection, Maintenance, and Repair of Drilling and Well Servicing Structures (API Recommended Practice 4G, 2004), Section 13.1.
