Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and burn injuries while working from the platform of the "old man" during the removal of unstable electrodes: (a)#15 Submerged Arc Electrode Furnace: Employees are exposed to struck-by and burn injuries while performing carbon change due to unstable head and electrodes. An employee was struck and pinned by a head and electrode assembly that fell over onto the "old man" platform, as determined on or about July 28, 2009. Among other methods, one feasible and acceptable abatement method to correct this hazard is to eliminate the possibility of an unstable head and electrode by developing and ensuring use of work procedures which will support and stabilize the heads and electrodes through visual inspection and mechanical means.
