Section 5(a)(1) of the Occupational Safety and Health Acts of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards: a.Drilling Site. Employees were exposed to the hazard of collapse of the derrick/mast in that the rig contractor did not ensure that anchor capacities were verified prior to attaching guylines to the deadman anchor/stakes for the Rambler Rig #21, serial # 689. Employees were performing work on the rig floor and pipe racking board, as determined on or about 04/17/08. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS THAT THE CAPACITY OF THE ANCHOR SHALL BE VERIFIED BY PULL TESTING OR OTHER METHODS BASED UPON ACCEPTED ENGINEERING PRACTICES THAT YIELD EQUIVALENT RESULTS TO PULL TESTING, AS SPECIFIED IN THE RECOMMENDED PRACTICE FOR USE AND PROCEDURES FOR INSPECTION, MAINTENANCE, AND REPAIR OF DRILLING AND WELL SERVICING STRUCTURES (API RECOMMENDED PRACTICE 4G), SECTION 14.5 (A), AND THE EMPLOYER SHALL BE RESPONSIBLE FOR PROVIDING ANCHORAGE CAPABLE OF WITHSTANDING THE MAXIMUM GUYLINE LOADS, AS SPECIFIED IN THE SPECIFICATION FOR DRILLING AND WELL SERVICING (API SPECIFICATION 4E) SECTION 3.14.
