Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by unexpected movement of the construction hoist counterweight: On or about 12/26/007 At the construction jobsite located at 50 W 15th St, New York, N.Y: Employees working on the Alimak Personnel/ Material Hoist were exposed to being struck by the moving hoist car, counterweights, or other parts of the hoist equipment. The employer failed to ensure that the car would not be operated if employee(s) were on the hoist mast or any location where they could be injured by moving hoist car(s), counterweights or other moving parts of the hoist equipment. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: a) Follow the Alimak Operation & Maintenance Manual, page C-2, which states; "Under no circumstances the hoist, single or dual, shall be driven if there is a person within the ground enclosure, on the mast or tie". b) Always lock out the main "ON/OFF" switch to prevent unexpected operation while service / inspection work is carried out. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19
