Text For Citation: 01 Item/Group: 001 Hazard: LIFTING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of structural collapse and failure of the crane and critical components: a)In the workplace: On July 18, 2008 and times prior thereto, the employer did not ensure that periodic inspections, were being conducted on the R3 rig, used to install water irrigation pumps and associated piping weighing approximately 15,000 to 20,000 pounds. b)In the workplace: On July 18, 2008 and times prior thereto, the employer did not ensure that a rated load capacity was determined, including but not limited to loads which may be imposed laterally from side loading, for the R3 rig prior to putting it into service. Note 1) Instance a) Feasible and acceptable abatement to correct this hazard is to comply with ANSI B30.5-1968 Crawler, Locomotive and Truck Cranes, section 5-2.1.3 Periodic Inspections, and/or ANSI B30.6-1969 Derricks, section 6-2.1.3 Periodic Inspections, which include, but are not limited to parts such as pins, bearings and shafts for wear, cracks and distortion, with a written, dated, and signed record of the inspection results. Note 2) Instance b) Feasible and acceptable abatement to correct this hazard is to comply with ANSI B30.5-1968 Crawler, Locomotive and Truck Cranes, section 5-2.1.1 Load Ratings-Where Stability Governs Lifting performance, and/or section 5-1.1.2 Load Ratings- Where Structural Competence Governs Lifting Performance. Note 3) Abatement certification and documentation is required for this item.
