Text For Citation: 01 Item/Group: 001 Hazard: CHEMICAL
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing of likely to cause death or serious physical harm to employees in that employees were exposed to thermal burns, chemical burns, and severe skin irritation from heated asphalt products and emulsifiers during transfer processes. 2200 Avenue H E, Omaha, NE: On July 13, 2009, and at times prior to, employee was exposed to heated asphalt product, approximately 240 degrees Fahrenheit, during the transfer process from a railcar to a storage tank. The system experience an over-pressurization, causing a disconnection between the banded transfer hose and the coupling. Heated asphalt was released from the system, covering the employee. Feasible means of abatement may include, but are not limited to, the following: 1)Using crimped or permanent hose fittings for product transfer. 2)Following the manufacturer's recommendations for maintenance and storage of equipment, pumps and hoses. 3)Conducting periodic inspections and testing of equipment, pumps and hoses. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
