Text For Citation: 01 Item/Group: 001 Hazard: UNCLASS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of: a) On or about June 23, 2008, an employee was fatally injured when he was thrown from a utility vehicle cargo box while being transported throughout the tank range. The employee was thrown from the cargo box after the utility vehicle reached the turn pad of the battle position striking his head on the concrete turn pad. The utility vehicle was clearly marked with several warning signs located on the utility vehicle that prohibited the use of the cargo box as a means to transport personnel. Among other methods, recognized and feasible means to correct this hazard include, but are not limited to : not allowing ( prohibit ) the use of the cargo box of the utility vehicle/ specialty vehicle to transport personnel. Train employees on the safe use of the utility vehicle/ specialty vehicle and on the hazards of the workplace. Comply with Army Regulation 385-10; contractors shall develop a written site specific plan, designate a quality control person to be responsible for safety and health at the work site. Conduct regular safety training for the workforce, conduct frequent and regular inspections for compliance. Operators of utility/ specialty vehicles shall be subject to a qualification and training program. Possess a military operators permit. Contractors utilizing utility/ specialty vehicles shall not exceed personnel capacity. Approved head protection (helmet) shall be provided to operators and passengers of utility/specialty vehicles.pecialty
