Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: (a) On or about July 27th, 2007, at approximately 3:00 pm, an employee was injured when a bolt that attaches the bales to the traveling block came loose and fell approximately 80 feet striking the employee on the head. A safety pin was not placed through the bolt to secure it from falling and the rig was not equipped with an anti-two block device to prevent the traveling blocks from striking the crown. Among other methods, feasible means of abatement to correct the hazards would be to: 1. Secure the bolt on the traveling block with a safety pin. 2. Follow the inspection protocol established in API RP 4G. 3. Equip the rig with an anti-two block device to prevent the traveling block from making contact with the crown block in accordance with API RP 54, section 9.4.8.
