Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5a1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards; (a) Coal Handling Area: Employees were exposed to struck by hazards due to lack of effective loader training. (b) Coal Handling Area: Employees were exposed to struck by hazards due to lack of visibility from the only source of lighting at night being inoperable and a shattered windshield, and lack of effective pre-shift inspection procedures. One feasible and acceptable method to correct this hazard would be to have all loader operators trained to the standard recommended by the provided Operation and Maintenance Manual located inside the loader,to follow requirements on the CATERPILLAR Safety and Maintenance Inspection check list and take equipment out of service that does not meet the applicable requirements until it is repaired. Maintain lighting so employees working on night shift will have visibility. Abatement Note: Abatement certification and documentation is required for this item.
