Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and struck by hazards; (a) Fabrication Yard: On or before September 10, 2009 an employee was exposed to the hazard of falling a distance of up to approximately 40 feet to the concrete below, and employees working under the elevated platform were exposed to being struck by the falling platform due to improper rigging of the U-bolts used on the wedge socket attached to the live line. One feasible and acceptable method to correct this hazard would be to follow the instructions found in the ANSI B30.5-2000 Section 51.7.3 (Mobile and Locomotive Cranes), regarding the proper rigging for a wedge socket.
