29 CFR 1910.147(c)(1): The employer did not establish an energy control program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative: a) On or about June 2, 2009 and times prior, the employer failed to develop an Energy Isolation (Lockout/Tagout) program exposing employees to serious injuries while performing work on equipment such as, but not limited to, the API waste water system at the Montana Refining Company, Inc.
